New Patient Registration

Steps to New Patient Registration

Step 1
Check you are within the practice's catchment area

Each practice has a catchment area within which they can accept new patients.

Step 2
Visit the practice

You need to come into the practice to make an appointment for a new patient check with our nurse. Appointments for new patient checks cannot be made over the phone, you must attend the surgery in person (special circumstances will be considered). You will be given a registration form and a new patient questionnaire to complete in the surgery and hand back in to reception.

Please note: You will need to bring photographic ID and proof of address to register with the practice.

Step 3

Complete Registration Form
The registration form will ask you for personal details such as your name, address, date of birth, telephone number, previous address and doctor. This information helps the Primary Care Trust to quickly trace your medical records and send them to your new surgery. The New patient questionnaire will ask you for some basic information about lifestyle and ethnicity, so we can provide the best service possible from the start.

Step 4

Attend new patient check.

It is important that you attend your appointment with the nurse for a new patient check. She will take a medical history from and enter any significant information such as allergies and medications onto your computer record.

Step 5

Placed on the Practice list
Once you have completed and signed the form and attended your new patient check, you will be registered with the practice. Shortly after this the PCT will send you a new medical card with your doctor's name on for you to keep.
